NYS DOH Requests Applications for Health Care Facility Transformation Program

The New York Department of Health (DOH) recently announced it is accepting applications for the Statewide Health Care Facility Transformation Program IV. The grants will distribute $250 million to support capital projects and debt retirement and restructuring, as well as working capital or other non-capital projects directly related to a capital project that facilitate furthering transformational goals. Those goals include, but are not limited to, transforming, redesigning and strengthening quality health care services to align with:

  • statewide and regional health care needs and the ongoing pandemic response
  • other activities intended to build innovative, patient-centered models of care to increase access to care, improve the quality of care and to ensure financial sustainability of healthcare providers.

The announcement reports that grants will not be made to support general, day-to-day operating costs.

DOH reports that eligible applicants include hospitals, residential care facilities, adult care facilities, assisted living programs, children’s residential treatment facilities licensed by the Office of Mental Health (OMH), and residential facilities or day program facilities licensed by the Office for People With Developmental Disabilities (OPWDD). Eligible applicants also include community-based health care providers which include diagnostic and treatment centers, home care providers, primary care providers, independent practice associations or organizations, hospice providers, outpatient clinics licensed by OMH, the Office of Addiction Services and Support (OASAS) or OPWDD, and community-based programs funded by OMH, OASAS, OPWDD or through local governmental units.

Applications are due on or before March 26, 2024, at 4 PM.

Questions are due by Feb. 20. 2024, with answers posted on or about March 5, 2024. It is expected awards will be made on July 1, 2024, for a five-year contract length.

Statewide Health Care Facility Transformation Program IV – Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care

This grant opportunity seeks to award $50 million to residential and community-based alternatives to the traditional model of nursing home care. The announcement identifies two transformation programs (Kings County Health Care Transformation Program and Oneida County Health Care Transformation Program) that will not be eligible under this grant.

Applications are due on or before April 9, 2024, at 4 PM.

Questions are due by Feb. 14, 2024, with answers posted on or about March 5, 2024. It is expected the awards will be made on July 1, 2024, for a five-year contract length.

Statewide Health Care Facility Transformation Program IV and V Health Information Technology, Cybersecurity and Telehealth Transformation

This grant opportunity seeks to award grants to support technological and telehealth projects that facilitate health care transformation activities. Funding will be provided as grants and can include capital, non-capital and working capital expenses. Proposals should focus on one of the four identified categories:

  1. Electronic Health Records
  2. Cyber Security
  3. Health Management Tools
  4. Telehealth

Applications are due on or before March 13, 2024, at 4 PM.

Questions are due by Jan. 26, 2024, with answers posted on or about Feb. 9, 2024. It is expected the awards will be made on July 1, 2024, for a five-year contract length.
