On August 21, 2024, Pomona Community Health Center d/b/a ParkTree Community Health Center (“ParkTree” or “PCHC”) filed a notice of data breach with the Attorney General of California after discovering that an unauthorized party was able to access information stored on the company’s computer network. In this notice, ParkTree explains that the incident resulted in an unauthorized party being able to access consumers’ sensitive information, which includes their first and last names, dates of birth, health plan identification numbers, medical record numbers, contact information, genders, and medical information. Upon completing its investigation, ParkTree began sending out data breach notification letters to all individuals whose information was affected by the recent data security incident.

What Caused the ParkTree Community Health Center Data Breach?

After learning that sensitive consumer data was accessible to an unauthorized party, ParkTree Community Health Center reviewed the compromised files to determine what information was leaked and which consumers were impacted. PCHC completed this process on July 9, 2024.
